kevinveenbirkenbachandClaude Opus 4.7 3387aa344f refactor(svc-db-postgres): per-consumer extensions declared in meta/services.yml via service_extensions lookup

Each consumer (bookwyrm/mobilizon/discourse) now declares its required
postgres extensions in its own meta/services.yml under postgres.extensions
and pins shared: true. svc-db-postgres' 02_init.yml drops the hardcoded
postgis-gate and unconditional vector steps and loops over the new
lookup keyed by database_consumer_id. The lookup fast-fails if a role
declares extensions without pinning shared: true.
