Replace the single docs/agents/action/debug.md with a debug/ directory
hosting three distinct workflows: local-deploy debugging, fully
automated CI pipeline triage via gh, and inspection of operator-placed
log files. Add full pipeline triage flow (enumerate failed runs and
children, classify branch-caused vs external, dedup-then-create issues
on s.infinito.nexus, iterate per role batches with APPS cap, push the
fix branch, file follow-up issues for unresolved root causes).

Use this page when the operator has manually placed *.log or *job-logs.txt files in the repository workdir for ad-hoc inspection. For the fully automated CI triage that fetches logs itself via gh, see pipeline.md.

You MUST inspect the relevant logs in the provided *job-logs.txt or *.log files.
You MUST treat the provided log files as the source of truth for the failure they describe.
